- Conduct thorough evaluations in both inpatient and outpatient settings, creating customized treatment plans specifically designed for each patient
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functional capacity
- Collaborate closely with nurses, physicians, case managers, and fellow rehabilitation professionals to optimize patient outcomes
- Provide education to patients and their families regarding therapeutic objectives, discharge procedures, and home exercise regimens
- Keep meticulous records of evaluations, treatments, and progress in accordance with both facility and regulatory standards
- Engage in care team meetings and interdisciplinary discussions to ensure seamless, coordinated patient care
- A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited educational institution
- Valid state license as a Physical Therapist
- BLS/CPR certification preferred
- Additional specialized certifications (e.g., geriatric, neurological) are advantageous but not mandatory
- Previous clinical experience is highly preferred
- Ability to work with patients experiencing a variety of complex medical and mobility challenges
- Exceptional communication and cooperative teamwork abilities
- Comfortable utilizing digital documentation and clinical charting systems
