Case Manager

Job Summary:

The Housing Case Manager is responsible for providing comprehensive case management services to individuals and families experiencing or at risk of homelessness. This role involves helping clients secure and maintain stable housing, connecting them with community resources, and supporting their progress toward long-term self-sufficiency. The ideal candidate has strong advocacy, problem-solving, and communication skills, with experience working with vulnerable populations.

Key Responsibilities:
  • Conduct intake assessments to determine housing needs and program eligibility.
  • Develop individualized housing and service plans in collaboration with clients.
  • Assist clients in securing housing, including completing applications, advocating with landlords, and overcoming barriers such as poor credit or past evictions.
  • Provide ongoing case management, including regular home visits and check-ins.
  • Coordinate supportive services such as employment assistance, mental health counseling, and substance abuse treatment.
  • Maintain accurate and timely documentation in case files and databases.
  • Monitor clients' progress, adjusting service plans as needed.
  • Mediate landlord/tenant issues and help clients comply with lease terms.
  • Participate in team meetings, training sessions, and community outreach events.
