Hospice Nurse Practitioner — Palliative Care Leader

At Home Hospice, part of the UnitedHealth Group family, is seeking a Hospice Nurse Practitioner to provide consultation in palliative and supportive care for hospice patients as requested by the referring physician. You will perform face-to-face visits, coordinate plans of care, and collaborate with the Hospice Medical Director and interdisciplinary team to meet patient and family needs.
