Home Infusion Care Transition Liaison

InfuCare Rx seeks a Care Transition Coordinator to support patients needing home infusion services in South New Jersey. Role requires travel within the territory and collaboration with hospital discharge planners, physicians, and home care teams to coordinate timely referrals and ensure seamless discharge planning. You will assess patient needs, gather referral information, educate patients on services, and help navigate insurance coverage while maintaining HIPAA compliance.
