United Transitional Care Center, the hospital-based skilled nursing facility located on the fifth floor at United Hospital Center, provides the right place for many older patients to receive supervised restorative care after hospitalization or an acute illness.

It provides the right care at the right time — when the patient has begun recovery but is not yet well enough to be home.

The hospital-based skilled nursing facility bridges the gap between high-tech hospitalization and family home care.

Our specialty is geriatric nursing.

We focus on improving the patient's health skills and ability to function within the community.

Because the hospital-based skilled nursing facility is affiliated with an acute-care hospital, each patient receives optimal care from the full range of health care disciplines.

Medicine, nursing, respiratory therapy, and other departments contribute to the patient's progressive restoration to the highest achievable level of functioning.
