- Telehealth services available
Provider Requirements:
- Nurse Practitioner or Physician Assistant
- Board Certification preferred
- Minimum of 2 years of recent clinical experience
- Pediatric experience
- FQHC or community health experience required
- Active California license or ability to obtain
- Prior Medicare/Medicaid enrollment required
Credentialing:
- Credentialing timeframe: 60–90 days
- Credentialed with major payors and insurance carriers
