Job Summary:

We are seeking a dedicated Physical Therapist to provide rehabilitative services in a Skilled Nursing Facility setting. The role involves assessing patient needs, developing treatment plans, and implementing therapies to improve patient mobility and functional ability.

Responsibilities:

• 
  Evaluate patients' physical conditions and develop individualized treatment plans.
  
  
• 
  Administer therapeutic exercises and manual therapy techniques.
  
  
• 
  Monitor and document patients progress, adjusting treatments as necessary.
  
  
• 
  Collaborate with healthcare team members to coordinate patient care.
  
  
• 
  Educate patients and their families on therapy goals and home exercises.
  
  
• 
  Maintain clear, accurate records in compliance with regulatory standards.
  
  

Qualifications:

• 
  Hold a valid state license as a Physical Therapist.
  
  
• 
  Possess a degree from an accredited physical therapy program.
  
  
• 
  Have experience working in a SNF or similar post-acute care setting.
  
  
• 
  Demonstrate strong interpersonal and communication skills.
  
  
• 
  Be able to lift, position, and handle patients safely.
