About the Opportunity

We are seeking experienced Nurse Practitioners to conduct in-home Annual Wellness Visits and chronic condition assessments. This is an ongoing PRN opportunity designed for NPs seeking flexible scheduling and reliable additional income .

Responsibilities

• Conduct in-home wellness and chronic care assessments
• Perform focused histories, physical exams, vitals, medication reviews, and screenings
• Provide patient education on preventive and chronic disease management
• Complete accurate documentation in the EMR

Qualifications

• Active, unencumbered New York NP license
• 3+ years of patient care experience preferred
• Comfortable working independently and using EMR systems
• In-home or risk-adjustment experience preferred

Role Expectations

• Minimum 30 hours per month , ongoing
• Consistent availability and reliable completion of scheduled visits
• Daily travel within a 55-mile radius of the assigned county
