PRN-Licensed Practical Nurse

Village Caregiving

Cheyenne is seeking an experienced and compassionate PRN Licensed Practical Nurse to join our exceptional home care team in the Cheyenne (Laramie, Wheatland, Torrington) Area. Village Caregiving is a non-medical home care agency that provides caregivers to assist clients with ADLs from the safety of their homes. The primary focus of the LPN is to visit client's homes to document medical history and complete health assessments both in-person and over the phone when directed by the Director Of Nursing. The LPN will assist with developing a personalized plan of care for our caregivers to follow based on each client's individual needs. This is a flexible position, and assessments can be completed during the day or evening hours, and weekends if preferred (as scheduled with clients). Pay is $50 per in-person assessment completed and $25 per over the phone assessment completed.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify the physical, psycho-social, and environmental needs of home health patients
• Assess patients conditions and chart observations, and perform evaluation tasks including vital signs and medication review
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patients quality of life
• Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Qualifications

• Current state license as an LPN
• Compassionate, friendly demeanor
• Excellent communication skills
• Current drivers license and reliable transportation
• Ability to work a flexible schedule and travel locally
