Veterans Case Manager: Trauma-Informed Care & Support

Volunteers of America

Volunteers of America Colorado seeks a Case Manager 1 to provide community-based case management and care coordination for Veterans and military families in the SSG Fox SPGP program. The role focuses on outreach, eligibility assessments, and connecting participants to VA and community resources.

You will develop individualized service plans, promote stability and well-being using trauma-informed practices, document interactions, and collaborate with VA centers and local providers.
