Newport News, Virginia Overview The Speech-Language Pathologist will follow appropriate practices to perform evaluations, timely re‑evaluations and OASIS assessments, determine SLP diagnoses, develop a plan of care, and perform SLP procedures to aid in restoring/maintaining speech, language, motor, and swallowing abilities. This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. The Therapist is required to be flexible with daily scheduling and may be required to work various patient care settings. May need to assist in coordination of Home Health Aides, participate in interdisciplinary case management, complete quality chart reviews, participate in meetings as required by the facility/department, and engage in other initiatives to improve quality outcomes. What You Will Do Perform evaluations, determine SLP diagnoses, develop plan of care, and perform speech therapy procedures to aid in restoring/maintaining speech, language, motor, and swallowing abilities. Evaluate and assess the patient in a timely manner based on principles outlined by practice guidelines, evidence‑based guidelines, and regulatory standards. Provide reassessments including OASIS assessments at appropriate time points to determine the level of functioning, and update and revise the plan of care in consultation with the physician and interdisciplinary team members. Provide therapeutic treatments and evaluation of equipment needs as applicable, prepare clinical and progress notes that support skilled intervention and assess outcomes of care, and plan for discharge of patient. Build trusting collaborative relationships with a multi-disciplinary team to support patient care needs, actively participate in multi-disciplinary team meetings, in-services, and case conferences. Carry out treatment plan effectively and modify to maximize therapeutic benefit and meet discharge criteria and return to optimal level of care, utilizing testing results and developing updated plan of care based on principles outlined in appropriate evidence‑based guidelines. Coordinate with Home Health Aides in providing services including completion of required supervisory visit documentation, and participate in training and support of all team members, volunteers, and students. Qualifications Education Masters Degree in Speech Language Pathology (Required) Experience 1 year Speech Language Pathology experience (Preferred) Licenses and Certifications Speech Language Pathologist (SLP) - The American Speech-Language-Hearing Association (ASHA) Upon Hire (Required) Speech Language Pathologist (SLP) - Virginia Department of Health Professions (VDHP) Upon Hire (Required) CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days (Required) Certificate of Clinical Competence in Speech Language Pathology (CCC-SLP) - The American Speech-Language-Hearing Association (ASHA) Upon Hire (Required) Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job Valid Driver's License (Required) #J-18808-Ljbffr
