Requirements
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance

Roles & Responsibilities
- Assess patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families on care, medications, and home safety, promoting self-care practices.
- Supervise and provide clinical direction to nursing aides to ensure quality service.
- Monitor and document patient pain management and make necessary adjustments.
- Communicate effectively with patients, families, and interdisciplinary team members to coordinate care.

Compliance / Notes
- Performs on-call responsibilities and provides on-call services as assigned.
