HouseCalls is a unique, outcomes-driven program that delivers annual in-home health assessments to Medicare Advantage and other health plan members. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) , you'll focus on preventive care, patient education, and collaboration with primary care providers to improve overall health and wellness.

This is a non-prescribing, field-based role that allows you to spend meaningful time with patients while enjoying the autonomy and flexibility that comes with a mobile care model.

• Conduct comprehensive in-home health assessments, including:
  • Medical history review
  • Medication reconciliation
  • Vital signs and physical examinations
  • Evidence-based screenings and point-of-care testing, as appropriate
• Identify and document diagnoses to support care management and future treatment planning
• Partner with primary care providers by communicating findings and helping address gaps in care
• Recognize urgent or emergent situations and take appropriate action
• Educate members on disease management, medications, and treatment adherence
• Assess social determinants of health and connect members with community resources when appropriate
