Description: Candidates must have prior recent experience in case management. They will be responsible for managing care coordination and utilization review. They will complete initial assessments and discharge planning. Candidates will interact with various payers and address needs of the uninsured or homeless. Effective communication and teamwork are essential skills for success. They must demonstrate the ability to prioritize tasks throughout the day. Knowledge of Cerner charting system is required. Orientation consists of unit-specific training based on the candidate's experience. Weekends are required based on staffing needs, with at least one full weekend worked per month. Holidays are not required, but candidates should be willing to work if needed. This position involves collaboration with hospital staff and may include working with hospitalists or intensivists. Candidates will be trained in utilization management protocols.
