Schedule: Day Shift - Remote

Assignment Details: Holiday Coverage Required: No

Requirements: 1 year of experience required

Responsibilities:

• Collaborate with physicians, nursing staff, residents, and families to ensure the effective use of the Minimum Data Set (MDS) assessment tool.
• Facilitate the development and implementation of resident care plans.
• Coordinate and participate in interdisciplinary team meetings.
• Ensure compliance with all relevant Federal and State regulations and facility policies.
• Provide individualized nursing care for residents in skilled nursing facilities.

Compliance / Notes: Details related to orientation, background checks, and other logistics will be provided during the onboarding process.
