Occupational Therapist OT Home Health (PRN) Job Category : Allied Therapists Requisition Number : OCCUP013589 Posted : May 11, 2026 Part-Time Locations Showing 1 location Empath Home Health , a member of Empath Health is seeking a compassionate PRN Home Health OT (Occupational Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in east Pasco County (Wesley Chapel, Odessa, Zephyrhills, Land O' Lakes, and Dade City). Locations: Home health visits in east Pasco County (including Wesley Chapel, Odessa, Zephyrhills, Land O' Lakes, Dade City , and surrounding areas) Schedule : PRN role; 5-10+ home health visits per week. What You’ll Do Perform comprehensive patient assessments, including OASIS assessments at required intervals Develop, initiate, and revise individualized plans of care as patient needs evolve Deliver skilled occupational therapy services and treatments as outlined in the plan of care Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies Collaborate with the interdisciplinary team to ensure seamless, holistic care Why Join Empath Health as a PRN clinician? What You’ll Need Minimum of 1 year experience as a licensed Occupational Therapist (OT). Home health care experience strongly preferred.
