Physical Therapist (PT)

Role Overview: Responsible for coordinating and delivering physical therapy services, including evaluating patients, developing and implementing individualized treatment plans, and providing direct therapy in various settings. Ensures effective communication with patients, families, and interdisciplinary team members to promote health and achieve treatment goals. Key Responsibilities Evaluate patients using multiple data sources. Develop and review personalized treatment plans, instructing patients and caregivers as needed. Use therapeutic activities to promote health and adjust interventions based on ongoing assessments. Provide therapy services aligned with patients' physical, emotional, and intellectual needs. Document evaluations, treatment plans, progress notes, and reports. Communicate effectively with the healthcare team and participate in care conferences. Oversee and collaborate with therapy assistants, techs, and students. Maintain knowledge of equipment use and ensure safe patient care. Qualifications & Skills Active license as a Physical Therapist in the practice state. Ability to perform essential functions, including good visual acuity and communication skills. Commitment to professionalism, confidentiality, and safety standards.
