Youth Care Coordinator (YCC)

Position Summary Bridgeway Center, Inc. (BCI) seeks applicants who want to work with clients to effectively address clients' care by assessing needs, developing, monitoring, and facilitating interdisciplinary approaches. The Youth Care Coordinator (YCC) manages an assigned caseload of clients having mental illness diagnoses, assessing client needs, developing, implementing, and reviewing service plans, and working with other community resources to meet or achieve client service needs. The YCC works with clients in the judicial system. The Care Coordination Program (CCP) provides services that help individuals link to needed medical, social, educational, psychiatric, psychological, and other services, including direct assistance in accessing services, coordination of care (CoC), oversight of the entire case, and linkage to appropriate services. Care Coordination involves assessment, development of an individualized integrated service plan, referral and related activities, monitoring and follow-up, and evaluation.
