Synergy Care is hiring a full-time Physical Therapist for an outpatient clinic serving an AL/IL community in Fort Worth, TX!
• Evaluate patients and develop individualized treatment plans
• Provide ongoing treatment and discharge planning
• Supervise assistants and aides
• Participate in case management
• Deliver high-quality occupational therapy services in accordance with state regulations and Synergy Care guidelines
• Represent Synergy Care's commitment to excellence in every patient and facility interaction Benefits Include:
