Social Worker III- Evening Shift

Position Title Social Worker III – Evening Shift (Job Number: 2602174) Position Summary At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to patients, their families, and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Responsibilities Psychosocial assessment of Social Work High Risk Criteria Cases. Coordinate with Medical Teams, Community Partners and Transitional Care teams. Charting and documentation including training, data management, statistics. Assist patient financial services, including Medicaid and other entitlement programs. Serve as liaison between community agencies and hospital. Preparation of material as needed for guardianship, oversight agencies, surrogate decision making. Prepare weekly updates for Long Stay Committee. Lead therapeutic groups when appropriate. Supervision of professional social work students when appropriate. Actively involved in performance improvement activities including program development, data collection and analysis. Other duties as assigned. Weekend coverage will be required. Required Qualifications MSW from an accredited Social Work Education School. NYS License within one year of hire. Experience in Medical Health Care. Excellent written and oral communication skills. Ability to work and coordinate with established Medical Treatment Team. Experience in discharge planning and Long Length of stay barriers and systems. Experience in Assessment and psycho-social skills. Working knowledge of Long Island and Metropolitan NY community resources. Ability to demonstrate patient first, patient advocacy and service excellence. Computer literate. Ability to implement and run medical team rounds. Understanding of community systems, project management and action planning, teamwork, leadership focus, data collection and statistics. Demonstrates ability to manage large and complex case loads. Understands role of SW and Case Management as it pertains to throughput and Length of Stay. Preferred Qualifications Experience in Multiple Medical Specialties in Social Work Health Care. Experience in Hospital, In-Patient setting, or skilled Nursing Facility. Discharge Planning experience. Diagnosis-specific Social Work. Bi-lingual. Benefits Generous leave, health plans, and a state pension. Location pay for UUP, CSEA & PEF full-time positions ($4,000). Inconvenience pay/supplemental shift differential ($575). Compensation Salary range: $86,162 – $109,275 Base. Schedule Full-time, Evening Shift (2:00–10:30 PM). Pass days: Saturday and Sunday.
