Social Worker - Licensed Clinical Social Worker

Social Services – LMSW

Assignments are available for a duration of 13 or 26 weeks with shifts of 5x8 hours or 4x10 hours.

Guaranteed hours: 72 hours per 2-week contract. The traveler can be called off for one 18-hour shift every two weeks.

Requirements

• LMSW (Licensed Masters of Social Worker) license
• Minimum of 2 years of acute care experience
• Experience with psychosocial assessments and discharge planning
• Ability to provide psychosocial/emotional support to patients and families
• Familiarity with EMR system: Epic

Responsibilities

• Provide psychosocial assessments and develop appropriate discharge plans.
• Assist with resource navigation and coordination of care.
• Engage in safety planning and screen for suicidality.
• Coordinate with interdisciplinary teams to ensure ethical and safe discharge plans.
• Complete necessary reports of harm based on regulations.
