Duties

Major duties include, but are not limited to:

• Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
• Administers medications and procedures per established policies and guidelines.
• Influences care outcomes by collaborating with members of the interdisciplinary team.
• Demonstrates leadership when providing, directing, and improving care for patients with end-stage renal disease (ESRD), acute kidney injury (AKI), and late-stage chronic kidney disease (CKD).
• Collaborates with other disciplines/departments to achieve desired patient outcomes.
• Functions as primary nurse for end-stage renal disease patient panel and serves as key resource assisting patients navigate the healthcare system.
• Completes documentation for all clinical and technical recordkeeping in the electronic health record and treatment flowsheets in accordance with established policies and procedures.
• Uses a team approach to identify, analyze, and resolve patient care problems with measurable outcomes.
• Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.

Pay: Competitive salary, regular salary increases, potential for performance awards
