Our facility offers 119 in-patient beds and the largest freestanding pediatric ventilator program in the country. Additionally, VPF has expanded to include The Bancroft School at Voorhees Pediatric Facility, Voorhees Pediatric Facility Medical Daycare, and Voorhees Pediatric Rehabilitation Services. VPF continues to be recognized as a leader in pediatric healthcare, and in 2013 VPF received the highest accreditation awarded by The Joint Commission. Hours: Full Time, 40 hoursper week, Monday- Friday.
Compensation: $85,000-$105,000per year Reports to: Facility Administrator
PositionPrerequisites:
Required
- Current NewJersey Registered Nurse license in good standing.
- 1-2years of experience in long-term care or subacute rehabilitation.
- Proficiencyin electronic health records and MDS software systems.
- Strongknowledge of RAI Manual, PDPM, and NJDOH Long Term Care regulations.
- Exceptionalattention to detail.
- Stronganalytical and critical-thinking skills.
- Ability tomanage deadlines and multiple assessments simultaneously.
- Strongunderstanding of clinical documentation and regulatory standards.
- Effectivecommunication and collaboration with interdisciplinary teams.
- Certificationas a Resident Assessment Coordinator (RACCT).
- Experiencewith NJ Medicaid Case Mix Index (CMI) system.
- Priorleadership or staff education experience.
- Occasionallifting, bending, or assisting during resident assessments.
- Completeand submit MDS assessments accurately and within required federal and statetimelines.
- Coordinateand lead the Resident Assessment Instrument (RAI)process.
- Maintaincompliance with CMS, NJDOH regulations, and facility policies.
- Initiateand complete Care Area Assessments (CAA)
- Ensuretimely completion of Significant Change in Status Assessments (SCSA)
- Analyze MDSData to identify opportunities for improvement
- Contributeto Quality Assurance and Performance Improvement (QAPI)initiatives.
- Develop andrevise individualized care plans based on MDS findings
- Ensureassessments support accurate Patient-Driven Payment Model (PDPM)
- Monitor theCase-Mix Index (CMI) for NJ Medicaid
- Serve asthe primary inter-departmental liaison, educator and supervisor of MDS relatedcare.
- Preparationand participation in NJDOH surveys, audits, and quality reviews.
