Director of Nursing (DON)

Position Summary The Director of Nursing (DON) leads all clinical services at Northern Oaks Place to ensure residents receive safe, personalized, and compassionate care. This role oversees assessments, care planning, medication management, infection control, documentation, staff competency, and regulatory compliance. The DON partners with leadership, staff, residents, and families to promote a high-quality, resident-centered environment aligned with Comfort Hills' values of Home, Care, and Comfort . Key Responsibilities Clinical Leadership & Care Oversight Ensure all resident assessments and individualized care plans are completed within state-regulated timeframes Oversee care plan updates following significant changes in condition Ensure residents are clinically approved and care-planned prior to move-in Maintain strong clinical outcomes through routine audits and quality monitoring Medication Management Ensure accurate, real-time documentation of medication administration in EMAR Maintain medication error rates below established thresholds and implement corrective actions as needed Ensure compliance with medication storage, disposal, renewals, and physician orders Incident Management & Resident Safety Ensure timely documentation, investigation, and follow-up of all incidents Maintain compliance with mandatory reporting requirements Identify trends and implement proactive risk reduction strategies Regulatory Compliance & Quality Assurance (QAPI) Ensure adherence to all clinical policies and state regulations Participate in QAPI initiatives and lead clinical quality improvement efforts Maintain accurate and complete clinical documentation through routine audits Address and resolve regulatory findings within required timelines Infection Control Oversee infection prevention and control practices Ensure staff training, compliance, and reporting requirements are consistently met Communication & Care Coordination Communicate changes in condition to physicians and families in a timely manner Ensure accurate and timely clinical documentation and care transitions Provide regular updates to residents and families regarding care needs Leadership & Team Development Lead and facilitate clinical staff meetings Participate in hiring, onboarding, coaching, and performance management of clinical staff Ensure staff competency through training, supervision, and annual evaluations Address performance concerns promptly and appropriately Resident Experience & Community Support Support resident engagement and participate in community activities as appropriate Collaborate with leadership to foster a positive, welcoming environment Support tours and provide clinical insight to prospective residents and families Qualifications Required Current, active Registered Nurse (RN) license in good standing Minimum of 2 years of clinical nursing experience (senior living, assisted living, long-term care, home health, or related setting) Minimum of 1 year of leadership or supervisory experience Experience with assessments, care planning, and change of condition management Knowledge of applicable state regulations Experience with electronic documentation systems (EHR/EMAR) Strong communication, critical thinking, and leadership skills Ability to work collaboratively with residents, families, and interdisciplinary teams Ability to meet the physical demands of the role Valid driver's license and reliable transportation Preferred Prior experience as a Director of Nursing, Health & Wellness Director, or similar leadership role Experience with QAPI or quality improvement initiatives Experience in memory care or dementia-related care Experience hiring, coaching, and developing clinical staff Work Environment & Physical Demands Works within a resident-centered senior living community environment Primarily on-site during business hours with flexibility for evenings/weekends as needed May participate in an on-call rotation Frequent walking, standing, and movement throughout the community Occasional lifting of up to 50 pounds Routine computer and documentation work Additional Information This position description reflects the primary responsibilities of the role but is not intended to be all-inclusive.
