About Caldore Acres

Caldore Acres Adult Day Health is a growing, person-centered adult day health program serving older adults, veterans, and adults with cognitive and physical disabilities. Our goal is to help participants remain independent while providing compassionate nursing care, engaging activities, and meaningful social interaction in a safe and welcoming environment.

We are looking for a compassionate Licensed Practical Nurse who enjoys building relationships and understands that every member of our team contributes to creating an exceptional experience for our participants.

IMPORTANT – This is a Working Nurse Position

This is not a desk nursing position.

When nursing duties are complete, our LPN works alongside the rest of our team to ensure participants receive the highest quality care throughout the day.

The ideal candidate is a team player who enjoys staying engaged with participants rather than remaining behind a desk.

Primary Nursing Responsibilities

• Medication administration and documentation
• Monitor participant health status
• Perform nursing assessments and observations
• Communicate participant concerns with families and providers when appropriate
• Maintain accurate nursing documentation
• Respond to medical emergencies
• Assist with care planning
• Ensure compliance with state regulations and company policies
• Maintain medication records and supplies

Team Responsibilities

When nursing responsibilities are complete, the LPN is expected to assist with daily program operations, including:

• Assisting participants with toileting and incontinence care
• Assisting with dressing and personal care as needed
• Participating in activities and encouraging participant engagement
• Assisting with meal service and hydration
• Escorting participants throughout the center
• Assisting with participant supervision
• Helping maintain a clean, organized, and safe environment
• Supporting fellow team members wherever needed

Qualifications

• Current Georgia LPN License in good standing
• Current CPR Certification (or ability to obtain)
• Excellent communication and documentation skills
• Compassionate, dependable, and team-oriented
• Ability to multitask in a fast-paced environment
• Experience working with older adults, dementia, or adult day health is preferred but not required

Physical Requirements

This position requires the ability to:

• Stand and walk for extended periods
• Bend, reach, and assist participants with mobility
• Assist with transfers using proper body mechanics
• Lift up to 35 pounds with or without reasonable accommodation
• Perform toileting and personal care assistance when needed
