Established to ensure that incarcerated individuals receive care that meets constitutional standards, CCHCS provides a range of services including preventative care, chronic disease management, emergency services, and substance use disorder treatment. Facility Address: 8260 Longleaf Dr, Elk Grove, CA 95758Primary Care Physician - ResponsibilitiesThe Primary Care Physician diagnoses medical conditions and order appropriate laboratory tests, imaging studies (e.g., X-rays), and other diagnostic procedures as needed.Collaborate with other agencies or health facilities on behalf of patients and their families to arrange admissions to hospitals, rehabilitation facilities, and skilled nursing facilities.Organize emergency ground and/or air medical transport as required.Analyze clinical data, develop therapeutic programs, and communicate information effectively to patients and their families.The Primary Care Physician provides support to patients and families in times of illness.Maintain accurate and timely Electronic Health Records (EHR) with detailed clinical documentation, including diagnosis codes.
