Family Home Health Network , an Affiliate of the Alden Network , is a provider of intermittent Home Health care, palliative care, hospice care, and skilled/custodial private care services throughout the Chicagoland area.
We are seeking a dedicated Home Health nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients' nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.
Minimum Qualifications:
Must possess current Illinois RN licensure
Home Health experience preferred
Current CPR
Valid IL driver’s license and auto insurance policy
HCHB EMR experience a plus
Must possess excellent clinical nursing and customer service skills
