Chronic Disease Care Navigator

The Chronic Disease Case Manager (CDCM) at Maniilaq Association in Kotzebue, Alaska, will promote health care delivery across the continuum to enhance quality of life, decrease fragmentation, and contain costs. The CDCM will support the Case Management Team and work under the Outpatient Services Manager.
