RN Case Manager - OPWDD

WCS Healthcare Partners is seeking a full-time Registered Nurse Case Manager to provide clinical oversight, case management, and patient care to individuals with Intellectual and Developmental Disabilities (I/DD) throughout Dutchess, Ulster, Columbia, and Westchester counties . This role combines clinical nursing expertise with individualized care planning in a supportive, mission-driven environment. New graduate RNs are welcome to apply!

Position Details

Location: Dutchess, Ulster, Columbia, and Westchester counties, NY
Schedule: Full-time, day shift
Start Time: Flexible start time of 7:00 AM, 8:00 AM, or 9:00 AM
Weekends: No weekends
On-Call: No on-call requirements
Population: Adults with moderate to profound I/DD

Key Responsibilities

Manage an OPWDD caseload of approximately 18–25 individuals, ages 20 through adulthood.
Travel between assigned residential group homes throughout the region to provide nursing oversight and support.
Conduct health assessments and monitor individuals' ongoing medical and nursing needs.
Develop and maintain individualized health and care plans.
Maintain accurate health records and ensure compliance with applicable developmental disabilities nursing standards.
Oversee and support medication administration by AMAP-certified staff.
Attend medical appointments as needed and collaborate with physicians, healthcare providers, and other members of the care team.
Participate in interdisciplinary team meetings and individualized planning.
Provide education, guidance, and clinical support to residential staff.

Minimum Qualifications

Current New York State RN license .
Strong communication, collaboration, and teamwork skills.
Compassion for individuals with I/DD and a commitment to providing high-quality, person-centered care.
Ability and willingness to travel between assigned residential group homes throughout the assigned counties.
Strong interest in collaborative nursing and individualized patient care.
New graduate RNs are encouraged to apply.
