Hospital Care Coordinator

SuperCare Health (SCH) is a post‑acute, in‑home respiratory care provider in the Western U.S. dedicated to serving the healthcare needs of our expanding patient population for nearly 50 years. We specialize in respiratory and chronic disease management and have recently launched a Diabetes division to expand our comprehensive healthcare services. Essential Duties Under the general supervision of the Patient Care Department Manager, the Hospital Liaison – Care Coordinator will cultivate strong relationships within multiple medical facilities and coordinate delivery of durable medical equipment (DME) and respiratory therapies. The role includes sales‑related responsibilities such as identifying referral opportunities, strengthening relationships with healthcare providers, and increasing our presence within assigned hospitals. Responsibilities Serve as the primary point of contact between SuperCare Health and hospital personnel, ensuring seamless coordination of DME and respiratory equipment deliveries to hospital rooms and patient homes. Develop and maintain strong relationships with physicians, case managers, discharge planners, and other healthcare professionals to generate new business opportunities and drive referrals. Promote and educate hospital staff on DME, respiratory therapies, and infusion services to increase awareness and referrals. Conduct regular facility visits to ensure visibility and accessibility for healthcare professionals and patients. Provide timely consultation to patients and their families regarding equipment delivery, potential co‑pays, and usage instructions. Request and ensure the accuracy of all necessary medical documentation for equipment orders. Monitor and track referrals, ensuring SuperCare Health is the preferred vendor for partnered healthcare systems. Identify opportunities to expand service utilization and sales within assigned hospital accounts. Conduct or participate in presentations, in‑services, and educational seminars for hospital staff and case managers to reinforce SuperCare Health’s value proposition. Maintain accurate records of interactions with hospital personnel, patient referrals, and sales activities, submitting required reports in a timely manner. Ensure compliance with insurance pre‑authorization requirements and coordinate with Managed Care and Primary Medical groups as needed. Maintain adequate inventory levels in assigned hospital storage areas to ensure prompt equipment availability and reconcile movement of equipment. Collaborate with Customer Care Specialists and Dispatch teams to optimize equipment delivery and customer satisfaction. Represent SuperCare Health professionally at all times and adhere to all HIPAA, company, and industry‑related regulations. Attend all department meetings and training sessions as scheduled. Perform all other duties as assigned by department leadership. Physical Demands Walk long distances across large healthcare facilities. Lift and carry equipment and miscellaneous items up to 75 lbs. on a daily basis. Work Environment Fast‑paced healthcare facilities with an emphasis on prompt deliveries and service coordination. High‑level communication and collaboration with multiple healthcare professionals and vendors. Manage multiple discharges, work under tight deadlines, and ensure efficient information transfer between SuperCare Health and hospital staff. Work Experience Requirements Experience in medical supply, home medical equipment (HME), or related healthcare industries. Strong knowledge of medical terminology and diagnosis codes. Proven experience in customer service and relationship‑building within a healthcare setting. Sales experience in a healthcare or medical‑related field (preferred). Ability to multitask, prioritize, and meet deadlines efficiently. Strong organizational skills with attention to detail. Benefits Medical, Dental, and Vision coverage.
