Per Diem RN Case Manager – Homecare VNA of Care New England welcomes compassionate and motivated RN case managers to join our Homecare team. This role offers summer flexibility while providing patient-centered, community-based care. What You’ll Do Perform comprehensive patient assessments and develop individualized care plans Coordinate care and referrals across disciplines and specialties Monitor patient progress and adjust care plans as needed Educate patients and caregivers on managing care at home Communicate effectively with patients, families, and healthcare providers Maintain accurate clinical documentation Support continuity of care and quality patient outcomes Why Join VNA Homecare?
