Key Responsibilities of the RN - Case Management - Travel

• Facilitate coordination of patient care and discharge planning.
• Assess patient needs and implement care plans.
• Screen new admissions for discharge planning needs.
• Coordinate with third-party payers for reimbursement.
• Participate in daily rounds and care conferences.
• Advocate for patient care and communication.
• Maintain knowledge of community resources for post-hospital care.
• Ensure adherence to standards and pathways during patient evaluations.
• Comply with infection control and safety standards.

Requirements of the RN - Case Management - Travel

• Current registration as a Registered Nurse in the Commonwealth of Massachusetts.
• Minimum 3 years acute care experience within the last 5 years.
• Working knowledge of InterQual or equivalent system.
• Experience in Discharge Planning and Utilization review.
• Bachelor of Science Degree in Nursing preferred.
• Certificate in Case Management or CPUM preferred.
• Strong interpersonal and organizational skills.
• Proficient computer skills.
