What You'll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing management. Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities. Manage pressure injuries, diabetic wounds, surgical wounds, skin integrity concerns, and related conditions. Coordinate care with physicians, nursing staff, therapists, caregivers, and community leadership. Support chronic disease management, preventative care initiatives, and continuity of care. Help improve clinical outcomes while reducing avoidable complications and hospitalizations. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license. Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing strongly preferred. Wound care certification is highly valued. Strong clinical judgment, professionalism, and confidence managing medically complex patient populations. Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
