Certified Medical Assistant

Position Summary Patient Navigator - Monday - Friday - 9am - 5pm - Brooklyn. The Patient Navigator supports the HIV Prevention Program, navigating patients through care services, benefits, treatment adherence, outreach, education, and referral services. The role may also include rapid HIV testing and pre‑ and post‑test counseling as required by grant and population served. Job Responsibilities Maintain a caseload of patients. Educate, coach and empower patients. Provide critical feedback to other members of the health care team. Provide translation services as needed. Coordinate navigation and logistical support for appointment reminders, transportation, and childcare arrangements. Perform entitlement reassessment and follow‑ups. Provide health education as needed. Provide skill‑building services to patients. Data entry and documentation. Assist with QI projects. Other duties as assigned. Minimum Qualifications Bachelor's degree or an equivalent combination of education and experience (e.g., Associate's degree and two years of experience; high school diploma/GED and four years of experience). Past experience in a hospital or health‑care setting preferred. Fluency in Spanish strongly preferred. Strong communication skills with all levels of the organization.
