Registered Nurse Case Management - Day Shift

Location: Eureka, CA

Assignment Duration: This is a 13-week contract position.

Schedule: Day Shift: 5x8-Hour (08:00 - 16:30). Required to work every other weekend.

Job Description: We are seeking a dedicated Registered Nurse with acute care experience for a Case Management position. This role will involve working in a dynamic healthcare environment, managing patient care, and coordinating with interdisciplinary teams to ensure the highest level of service delivery.

Responsibilities:

• Assess and manage patient care plans, ensuring quality and compliance with healthcare standards.
• Collaborate with other healthcare professionals to provide effective patient care.
• Act as a patient advocate to ensure their needs and preferences are met.
• Maintain accurate records and documentation in compliance with guidelines.
• Participate in orientation and training programs.

Required Experience / Certifications / Licensure:

• A minimum of 2 years of acute care experience.
• Current RN license for the state of California.
• Experience with Epic charting system is highly preferred.
• Charge experience is a plus, but first-timers accepted with clinical approval.
