This opportunity is well suited to a compassionate medical assistant who is passionate about improving the health and well-being of older adults through personalized, holistic support.ResponsibilitiesReview Community Health Worker (CHW) appointment documentation for accuracy and completeness, escalating issues to the nurse case manager as needed.Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers.Mail and/or email patient care plans and educational materials as directed by case managers.Accurately update and maintain patient records within the electronic health record (EHR) system.Protect patient confidentiality and support compliance with HIPAA and organizational policies.
