Job description:
- Setting: Skilled Nursing Facility, Sub-acute and long-term care patients.
- Schedule: Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions.
- Must be licensed in NY
- Experienced in SNF preferred, not required.
