Job description:
Setting: Skilled Nursing Facility, Sub-acute and long-term care patients.
Schedule: Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions.
Must be licensed in NY
Experienced in SNF preferred, not required.
