Job Summary Outpatient clinic – Patient panel – Primary Care – Geriatrics. Essential Duties and Responsibilities Coordinate multidisciplinary care delivered to panel of patients to optimize patient care. Review patient medical records to diagnose, treat, and provide care management and education to patients and families. Make appropriate patient referrals. Champion a culture of empathy, dignity, and respect for aging populations. Monitor national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for the management of patient care. Provide evidence‑based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Maintain a collaborative environment with other referring physicians, residents, medical students, advanced practice professionals, staff, and support staff to provide quality patient care. Support academic, educational and research endeavors. Assess and manage chronic and acute conditions in patients with multiple comorbidities. Partner and communicate with acumen to build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Multispecialty Group. Serve as a resource to the community. Perform other job‑related duties as assigned. Required Qualifications Current Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited school of medicine. Residency and/or Fellowship training in Family Medicine (Geriatrics). Current, unrestricted medical license to practice in the State of Texas. Board Certification or Board Eligible. Unrestricted work authorization to work in the United States. Preferred Education/Licensure/Certification and Experience Bilingual or multilingual proficiency is a plus. Prior experience in post‑acute or long‑term care settings strongly preferred. Proficient with electronic medical records and knowledgeable of CMS/SNF regulatory requirements. Key Competencies Comfortable working in a large public/county facility. Working knowledge in assigned area, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area. Good interpersonal, verbal and oral communication skills to work effectively with other health care professionals and serve as a resource to the community. Self‑directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area.
