About the Role Our client is seeking a compassionate and resourceful Community Health Navigator to join their dedicated team in Welkom . This role is central to assisting individuals and families in overcoming barriers to accessing essential healthcare services. You will act as a trusted liaison, guiding clients through the complexities of the healthcare system, connecting them with medical providers, social services, and preventative care programs. Your efforts will directly contribute to improving health outcomes and reducing health disparities within the community. Key Responsibilities Provide personalized guidance and support to clients navigating the healthcare system. Assist clients in scheduling medical appointments and understanding treatment plans. Connect clients with necessary health and social services, including mental health support and chronic disease management programs. Conduct outreach activities to raise awareness about available health resources and services. Educate clients on preventative health practices and healthy lifestyle choices. Maintain accurate client records and track progress towards health goals. Requirements Experience in community health, social work, or a related healthcare field. Strong understanding of the local healthcare landscape and available community resources in Welkom . Excellent communication, interpersonal, and motivational interviewing skills. Ability to empathize with clients from diverse backgrounds and varying health literacy levels. Proficiency in basic computer skills for record-keeping and communication.
