Licensed Clinical Social Worker (LCSW)STGi is currently seeking a Licensed Clinical Social Worker to provide services at our Community Based Outpatient Clinic.The Licensed Clinical Social Worker (LCSW) will provide social work services under the direction of the CBOC Primary Care Physician, Psychiatrist, Nurse Practitioner, and the VA Medical Center (VAMC) personnel.The essential functions of this position include:Providing social work service based on physician's orders to meet the needs (physical, mental, and emotional) of patients.Providing initial social work assessment for adult/geriatric Primary Care and/or Psychiatric patients.Participating in the completion of treatment, discharge planning, referrals and placements of patients as applicable.Providing coverage for chemically dependent patients as applicable.Providing psychotherapeutic intervention of a specialized nature to patients and or families having severe to complex psychosocial difficulties.Providing referrals for patients and families for individual, group, marital, family, financial or activity therapies.Providing mandatory reporting activities including Child Protective Services (CPS), Adult Protective Services (APS) and Elder Abuse.Providing crises intervention, supportive counseling and problem-solving interventions as applicable.Serving as a member of a multidisciplinary treatment team that meets to assess treatment needs, diagnosis and/or develop treatment strategies and goals.Reviewing progress of treatment(s) and providing appropriate feedback to a member of the multidisciplinary treatment team.Functioning as a primary liaison between families/friends of assigned patients, and between the VAMC.Serving as a community liaison with VA and Community Mental Health Centers and other appropriate agencies.Serving as a resource person to other CBOC staff members regarding other community services and programs.Ability to provide Group Therapy in accordance with state licensure and regulations.Maintaining universal precautions and infection control practices.Providing smooth and timely flow of patients in accordance with the VA access standards and triage protocol.Performing cardiopulmonary resuscitation (CPR) and assisting during respiratory and cardiac arrest procedures as applicable.Documenting all pertinent patient information in their medical records and VISTA to demonstrate quality care delivery and promote continuity of care.Maintaining the VISTA (Veteran Health Information Systems and Technology Architecture) clinic reminder tracking system current for each enrolled patient.Assisting in scheduling patients for initial and follow up appointments in accordance to the VA access standards and the patient's medical/psychosocial/psychiatric necessity.Maintaining patient information regarding their plan of care.Providing health educational materials and resources to patients and their families for informational purposes.Ensuring all patient information on access/wait time documentation is complete and accurate.Ensuring all patient information on electronic encounter documentation is complete and accurate.Participating in the ongoing Performance Improvement Program between Contractor, its Subcontractor(s), and the VAMC.Ensuring that all required documentation and reports are completed in an accurate, timely fashion and complete fashion.Assisting with the clinic's compliance of all Federal, state, local, JCAHO, OSHA, Veteran Affairs, Contractor VA Program and Subcontractor safety and operational regulations, directives and standards.Maintaining confidentiality of all information and support patients' privacy, rights, and safety.Performing other work related duties as assigned.Required skills include:Prefer one (1) year of experience in working with mental health/psychiatric patients.Prefer 2-3 years LMSW/LCSW experience.Current, full, active, and unrestricted license in Mississippi.Retain current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) from American Heart Association (AHA) or Military Training Network (MTN).Knowledge of and ability to apply professional social service principles, procedures, and techniques.Knowledge of crisis and family dynamics.Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations.Ability to set realistic, measurable education goals for patients, taking into consideration patient/family needs, level of understanding and available local resources.Ability to develop and provide individualized and group counseling, guidance, and problem-solving assistance.Knowledge of the basic concepts of customer service technique related to age-specific population.Demonstrated effective verbal and written communication skills along with proper telephone etiquette.Ability to demonstrate proficient usage of medical terminology.Working knowledge of Microsoft Office Software and basic computer maneuverability.Required education includes:Master's degree of Social Work from a college or university accredited by the Council of Social Work education.Psychotherapy certification required.Working conditions/physical requirements include:Moderate physical activity.
