Licensed Practical Nurse- Care Manager (LPN-CM)

Located in Florham Park, New Jersey 07932, the Licensed Practical Nurse- Care Manager (LPN-CM) facilitates patient continuity of care with the healthcare team. Under the clinical direction and oversight of the Primary Care Physician and Registered Nurse, the LPN Care Manager coordinates care for patients needing support by collaborating with the patient, family, physician, nurses, and other members of the healthcare team to identify needs and expedite valuable, appropriate, and cost effective care. The LPN-CM will support an individualized care plan focused on disease management and prevention, and or development of improved strategies to benefit patients at risk. They will engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, close gaps in care, and reduce unnecessary ER utilization and hospital readmissions.

Duties and Responsibilities

The duties include, but are not limited to:

• Implement and support individualized patient care plans
• Address patient risk factors and/or obstacles to care including hospital/ER utilization
• Facilitate engagement between PCP and patient
• Demonstrated ability to influence others while motivating them to change
• Participation in required care coordination conferences
• Maintain predetermined patient caseload
• Exercise sound clinical judgment, seek advice and sign off from Physician or Registered Nurse as appropriate
• Take initiative in finding solutions to difficult and/or sensitive problems
• Participate in quality gap closures
• Work as patient advocate
• Provide mentoring/coaching of other Value Based Program/Pop Health team members as assigned
• Performs miscellaneous job-related duties as assigned

Qualifications or Education, Training and Experience

• Must have current NJ Practical Nurse License
• Two (2)+ years of experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, or Home Health
• Must have valid driver license and ability to travel
