Job Summary The Life Skills Clinician (LSC) monitors and provides care coordination and life skills training and development in collaboration with the care team to youth and/or adults with mental health and/or substance use disorders, including those with serious and persistent mental illness and chronic addiction. Responsibilities Collaborates as part of a multidisciplinary care team member. Assist with gathering data for comprehensive psychosocial assessments. Deescalate clients displaying aggressive behaviors. Document services provided in Electronic Medical Record (EMR) based on policies and procedures. Transport clients in personal vehicles when necessary. Comply with all applicable Behavioral Health Care Service policies and procedures. Actively involved in professional development and education, including mandatory training. Complete time cards, PTO requests, mileage reports, and other documentation per Community Behavioral Health policies and procedures. Additional duties may be performed as needed; not all duties must be performed by every individual. Qualifications Bachelor's Degree in mental health or related field (required). Current Indiana Driver's License (required). Two (2) years of social service experience in a volunteer or paid role (preferred). #J-18808-Ljbffr Community Health Network
