Registered Nurse (RN) - Case Management

Acute Medical Care Manager – Care Coordination / Discharge PlanningTypical shifts are during the day, with a caseload of up to 25 patients. Weekend commitment typically includes one full weekend every four weeks, with variations noted.Up to 25 patients per day.RequirementsCalifornia state license required; pending acceptance for submission but must be licensed by start.At least 2 years of experience in acute medical care management, performing care coordination and discharge planning.Experience with electronic medical records (EMR), specifically Epic, is required.Familiarity with CMS/DRG/utilization concepts and HIPAA regulations.Strong assessment skills related to patient care plans and DME needs.ResponsibilitiesManage care coordination and discharge planning for a diverse patient population.Conduct patient assessments and develop comprehensive care plans.Collaborate with interdisciplinary teams to ensure quality patient outcomes.Provide education and resources to patients and families regarding discharge processes.Utilize computerized documentation for tracking care and outcomes.Must request all time-off upfront; no more than 7 days of requested time off at a time.
