Home Health Licensed Vocational Nurse (LVN) – Make a Difference Where It Matters Most About the Role BrightStar Care is looking for a compassionate, skilled, and dependable Home Health Licensed Vocational Nurse (LVN) to join our growing clinical team serving Quinlan, TX, and surrounding communities . As a Home Health LVN, you’ll have the opportunity to provide meaningful, one-on-one care to patients in the comfort of their own homes. This role is ideal for a nurse who values autonomy, clinical variety, relationship-building, and making a direct impact on patients and their families. You’ll provide skilled nursing care, monitor patient conditions, assist with clinical assessments, manage a variety of patient needs, educate patients and caregivers, and work closely with the RN, physicians, and interdisciplinary care team to support quality patient outcomes. What You’ll Do Clinical Care & Patient Assessment Provide skilled nursing care according to physician orders and the established plan of care Perform nursing assessments within the LVN scope of practice Monitor patient conditions and identify changes in health status Report changes in condition and clinical concerns to the supervising RN or appropriate healthcare provider Support patients recovering from hospitalization, managing chronic conditions, or requiring ongoing skilled nursing care Promote patient safety, independence, and quality of life in the home setting Skilled Nursing Services Provide a variety of skilled nursing procedures based on patient needs and physician orders Perform wound assessments and wound care , including monitoring healing progress and recognizing signs of infection or complications Provide catheter care and management , including urinary catheter care and patient/caregiver education Perform laboratory specimen collection and lab draws as ordered and within scope of practice Administer medications and monitor effectiveness and potential adverse reactions Assist with disease management and post-hospital recovery Provide infection prevention and control education Monitor medical conditions and promptly report changes in condition Care Coordination Coordinate care with the supervising RN, physicians, therapists, patients, caregivers, and other healthcare professionals Communicate patient progress, concerns, and changes in condition to the appropriate members of the care team Participate in care plan updates and interdisciplinary communication Follow the established plan of care and physician orders Patient & Caregiver Education Help patients and their caregivers feel confident managing care at home by providing education on: Disease processes and symptom management Medication management Wound care Catheter care Infection prevention Fall prevention and home safety Self-management strategies Medical equipment and device use When to report changes in condition Documentation & Compliance Complete accurate and timely electronic documentation Maintain thorough clinical records and visit notes Follow home health regulations, agency policies, and professional nursing standards Maintain HIPAA compliance and patient confidentiality Ensure care is provided according to physician orders and the established plan of care Practice within the scope of the Texas LVN license Qualifications Required: Current Texas Licensed Vocational Nurse (LVN) license in good standing Current CPR/BLS certification Minimum 1 year of nursing experience Strong clinical assessment and critical-thinking skills Excellent communication and organizational skills Valid driver's license Reliable transportation and current auto insurance Preferred: Previous home health experience Experience with wound careExperience with catheter care Experience with lab draws/specimen collection Knowledge of Medicare home health guidelines Experience with electronic medical records (EMR) Experience providing care independently in patients' homes What We’re Looking For We’re looking for an LVN who is: Compassionate and patient-focused Confident working independently in the home setting Organized and dependable Comfortable managing a variety of clinical needs A strong communicator and team player Committed to providing exceptional, high-quality care Comfortable collaborating with RNs, physicians, therapists, patients, and caregivers What We Offer Flexible scheduling Competitive pay Opportunities for professional growth and advancement Supportive clinical team Mileage reimbursement, if applicable The opportunity to build meaningful relationships with patients and families Work Location Patient homes throughout Quinlan, TX, and the surrounding service area .
