RN - Case Manager

Role Overview This registered nurse specializes in discharge planning, assessing patient needs, developing care transition plans, and collaborating with interdisciplinary teams to ensure compliance with discharge requirements. Key Responsibilities Ensure adherence to discharge planning standards, identify and address barriers to patient stays, assess high-risk patients for readmission, and develop strategies with the team for seamless care transitions. May also manage utilization, ED case management, admission processes, and liaise with post-acute providers. Qualifications & Skills Requires a nursing degree from an accredited program, three years of acute care experience, and active RN licensure in applicable states. Physical ability to perform clinical tasks, strong communication skills, and familiarity with evidence-based practices are essential.
