FULL TIME 40 HOUR (MIDNIGHTS 11:00PM TO 7:30 AM)

GENERAL SUMMARY:

As a Case Manager-Social Worker, you'll be at the heart of patient care, providing compassionate therapeutic intervention and social work services that transform lives. You'll work alongside patients and their families to create comprehensive, integrated, and uninterrupted care pathways—both within our hospital and throughout the community. Your role bridges clinical expertise with genuine human connection, ensuring every individual receives the support they need to heal and thrive. You'll demonstrate authentic, positive interpersonal communication with patients, families, visitors, and colleagues, embodying our organization's commitment to dignity and respect in every interaction.

KEY RESPONSIBILITIES:

 

• Conduct thorough psychosocial assessments to understand each patient's unique circumstances, strengths, and challenges, recognizing how social, familial, economic, and individual factors shape their wellbeing
• Develop and implement individualized care plans that address patients' holistic needs, collaborating with interdisciplinary healthcare teams to ensure coordinated, person-centered care
• Facilitate meaningful connections to community resources and services, advocating for patients and families to access the support systems they need for successful transitions beyond the hospital
• Provide compassionate counseling and therapeutic support to patients and families, helping them navigate complex healthcare decisions and life challenges with empathy and expertise
• Maintain thorough, accurate documentation and case records that reflect the patient's journey, clinical insights, and progress toward meaningful goals
• Collaborate proactively with hospital-based teams, community partners, and external agencies to build integrated care networks that prioritize continuity and patient outcomes
• Apply knowledge of child and adult protection laws, state Medicaid guidelines, and ethical standards to advocate for vulnerable populations and ensure compliance with professional standards
• Contribute to the organization's mission by providing clinical input on unit operations and participating in quality improvement initiatives that enhance patient care experiences
• Demonstrate cultural competence and sensitivity to diverse populations, recognizing and respecting individual differences in growth, development, and life circumstances across the lifespan

 

Qualifications

EDUCATION/EXPERIENCE REQUIRED:

Master's degree in Social Work required. Three (3) years of clinical experience. Knowledge of social work practices, individuals, case recording, chart documentation, community referrals, and effective networking techniques to coordinate services for patient outside of the hospital is required. Familiarity with general medicine psychiatry, psychology and sociology and how social, familial, economic, and individual factors affect how an individual functions required. Knowledge of child and adult protection laws, state Medicaid guidelines, NASW code of ethics (professional standards), and confidentiality and privacy, as applicable to the area of specialty is desired. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age-specific needs. Ability to make independent decision regarding clinical, psycho-social patient are and to provide input regarding the organization and functioning of the hospital unit required. Highly effective verbal and written communication skills are necessary to conduct successful counseling, social services, supervision of students, consultation with colleagues and documentation of patient counseling session. Strong collaboration, communication, and interpersonal skills. Excellent organization and time management skills. Knowledge of computers, Electronic Health Records, data base systems and utilization review/case management documentation systems. Desire to work collaboratively and proactively with healthcare teams and other hospital-based interdisciplinary teams. Knowledge of CMS, commercial payer requirements and hospital financial/ reimbursement processes desired. Excellent written/verbal communication skills, critical thinking skills, creative problem-solving skills, good organization, and planning skills. Must be self-directed, have the ability tolerate frequent interruption and work in a fast-paced work environment.

CERTIFICATIONS/LICENSURES REQUIRED:

Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license. Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association preferred.
