Nurse Practitioner OpportunityThe Charles B. Wang Community Health Center, an award-winning federally qualified community health center, is seeking a Nurse Practitioner for full-time coverage. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers.Responsibilities:Involve with direct patient physical assessment, education, and health planningMaintain continuity of care by regular patient follow-upAssist in the coordination of services with other disciplines and patients' insurance plansAdvocate for patient in arrangement of medically necessary servicesCommunicate with community resources to ascertain adequate assistance for patients in needRefer patients to specialty services when necessaryConsult with collaborating physician and other members of the health care team to establish plan of careParticipate in quality assurance process to assure optimal care deliveryProvide comprehensive case management services that include emphasis on health promotion and disease preventionMonitor patient outcome in accordance to the practice protocolsCollaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planningAct as a resource person for the nursing staffConduct education in service on practice issuesPerform other duties as assignedMinimum Qualifications:Graduated from approved Nurse Practitioner ProgramLicensed as Nurse Practitioner in New York StateBasic CPR (BCLS) certification requiredAt least 3 years of clinical experience as a primary care Nurse PractitionerBoard/Specialty Certification preferredBilingual in English and Mandarin Chinese requiredBenefits:Employer-paid dental, vision, disability and life insuranceEmployer-paid malpractice coverage through FTCAMedical insurance403(b) retirement savings planFlexible spending accounts for transit, parking, health and dependent carePaid time off (6 weeks)Continuing medical education time and allowanceEmployee Assistance ProgramEmployee Discount ProgramsNational Health Services Corps loan repayment program available, andThe Health Center is qualified for Public Service Loan Forgiveness (PSLF) program
