You will assess care plans, coordinate resources, and work with a multidisciplinary team to support high-risk patients across home, clinic, and facility settings. Requires LMSW or equivalent with CHW certification and 3–5 years of social work experience; knowledge of government resources and LIS/PAP/SNAP processes is essential.
