St. Luke Lutheran Community is seeking experienced, compassionate, and dependable Licensed Practical Nurses (LPNs) to join our close-knit nursing team at our beautiful Portage Lakes campus. Through our premier Weekend Warrior Program , you will enjoy premium pay rates while working in a smaller, lower-acuity facility backed by a highly supportive and collaborative team. What You’ll Do (LPN Responsibilities): As an LPN on our weekend team, you will deliver high-quality, patient-centered care to our residents. Key responsibilities include: Clinical Care: Administer medications, specialized wound care, catheter care, and other prescribed treatments. Resident Monitoring: Monitor, assess, and accurately document changes in residents’ health conditions. Care Planning: Assist in developing and implementing comprehensive, interdisciplinary resident care plans. Leadership: Supervise, support, and mentor State Tested Nursing Assistants (STNAs) on your shift. Collaboration: Partner closely with physicians, RNs, and the broader interdisciplinary care team to ensure seamless operations. Emergency Response: Swiftly respond to emergencies and acute changes in resident conditions. What We Are Looking For (Requirements): License: Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Ohio. Experience: Prior experience in long-term care (LTC) or skilled nursing is preferred. Skills: Strong clinical communication, sharp teamwork abilities, and a patient-centered approach to care. Pre-Employment Screening: Must be willing to complete a pre-employment physical and a BCI fingerprinting background check. Logistics: Direct deposit is required for payroll.
