Clinical Care Coordinator

Responsibilities The Clinical Care Coordinator participates as an integral member of the multidisciplinary care team at PMGA. Key roles are coordination of services for panel patients, focusing on those who face particular difficulty navigating the healthcare system, management of specialty referrals, support of patients in self‑care, and identification of relevant community resources. The Coordinator collaborates with and outreaches to outside service providers to provide seamless transitions of care, and provides patient education while assisting with access to applicable resources. Pay Pay Range: $23.81 to $36.44 per hour. Base pay may be supplemented by shift differentials, standby/on‑call, overtime, premiums, extra shift incentives and bonus opportunities.
